CCI Support Coordinator Position Description

OBJECTIVES:

The Coordinator promotes, develops and provides high quality assistance to individuals to support the role of people with a disability as valued, participating and contributing members of society. Coordinators do this by assisting people with disability to maximise their capacity to exert more choice and greater control over their formal and informal support arrangements. Predominantly – but not exclusively – funded supports are accessed through the National Disability Insurance Scheme (‘NDIS’) to deliver support or services to a participant of the NDIS. To a lesser extent some supports may be funded through ACT Government programs or philanthropic donations to Community Connections (CCI). Coordinators work within a rights-based framework of inclusion, consulting with the people we support about their needs, creating tailored plans which respond to the unique circumstances of each person, taking into account the funds available to enact support plans and coordination roles within available budgets.

MAJOR RESPONSIBILITIES AND TASKS:

  1. Assist people with disability, their NDIS nominees or legal guardians (as appropriate in each case) to exercise the maximum degree possible of choice and control over their funded support arrangements.
  • Assist people we support to develop their personal capacity and skills to attain greater autonomy and personal agency.
  • Provide high-quality support to people with disability and their families (where appropriate) designed to assist people to achieve a better life.
  • Interpret the NDIS rules and regulations and stay abreast of any changes or updates.
  • In consultation with participants and families (as appropriate) identify current goals and needs and create a personalised coordination of supports plan for the participant.
  • Work with the participant and their families to implement their plans – developing and maintaining relationships with support providers, sourcing equipment and arranging activities as per the plan outline and as required.
  • Liaise with representatives of the NDIS, attending annual planning meetings and unscheduled review meetings as required. Prior to meetings, ensure reports from therapists and medical providers are correct and up to date and that you are fully prepared and briefed on the participant’s needs and plan.
  • Assist participants to understand and direct their support plan funds, ensuring the budget is adhered to and funds are spent in accordance with funding rules and guidelines. Review progress against the participant’s budget regularly.
  • Undertake various administration tasks associated with management of the program including: –
    • Recording minutes from relevant meetings, ensuring minutes are saved in the relevant system;
    • Writing and maintaining case notes;
    • Completing purchase orders in line with individual budgets;
    • Collecting and maintaining statistical information; and
    • Other administration tasks as required. 
  1. Support participants and their families by assisting with the development of formal and informal support networks and developing activities and processes designed to improve the participant’s quality of life.
  1. Ensure the confidentiality and privacy of participants and their families. 
  1. Work within the fee-for-service environment of the NDIS to contribute effectively to sustaining Community Connections Inc., as a viable, values-based, charitable, not-for-profit, non-government organisation.

RELATIONSHIPS:

  1. Build strong, supportive and enabling partnerships with key stakeholders including:
    • people with disability, their NDIS nominees or legal guardians (as appropriate in each case);
    • colleagues at Community Connections Incorporated
    • representatives of external organisations: mainstream services, community organisations, funding bodies and relevant public, private sector and / or not-for-profit agencies.

GENERAL:

  1. Adhere to all organisation policies, procedures and guidelines, as amended, from time to time.
  • Assist team members when requested and request help where necessary.
  • Actively contribute to and participate in the professional life and activities of the teams at Community Connections Inc. Provide input into work practices and communicate issues.
  • Participate in the ongoing training and development of yourself and team members.
  • Assist in the identification of areas where improvements can be achieved.
  • Comply with all Work, Health and Safety Legislation as appropriate and ensure that all work is performed in a manner which does not place fellow employees or yourself in an unsafe position.
  • Carry out any other duties, as required and directed from time to time.

COORDINATOR SELECTION CRITERIA

Essential

  1. Qualification in the disability, health and/or community sector (minimum level Certificate 3 level).
  2. Demonstrated understanding and commitment to, the principles of social inclusion and a commitment to promoting the rights and independence of people with a disability.
  3. Direct experience working with people with disability.
  4. Understanding of the NDIS, including the NDIS planning and review processes and NDIS operational guidelines
  5. Excellent communication skills in building relationships with the people with whom we work with and their families. 
  6. Demonstrated ability to build productive relationships with a range of stakeholders.
  7. Strong problem-solving skills, accompanied by sound judgements.
  8. A high level of self-motivation and strong organisational and time management skills.
  9. Demonstrated ability to work independently and balance competing demands.
  10. Commitment to working in a team.
  11. Experience in using Microsoft Word and Excel, and web-based applications.
  12. Hold (or be willing to obtain) a First Aid Certificate including CPR.
  13. Hold a Working with Vulnerable People Registration/Worker Screening and current National Police Check.
  14. Required to undertake NDIA Worker Orientation Module.

Desirable

  1. Previous experience working as a Coordinator, Case Manager or similar.
  2. Current driver’s licence and own motor vehicle.
